What is Palliative Care?

If you or a loved one has been struggling with a chronic, life limiting illness which may be accompanied by uncomfortable symptoms (shortness of breath, pain, anxiety, etc), we at Anchor Health seek to help minimize that discomfort.  Our palliative program strives to improve your quality of life, assist with advance care planning, and offer support for additional resources within your community.  Palliative care is not the same as hospice care.  Often an individual chooses to continue with a more aggressive treatment plan for their medical condition recommended by their provider/physician.  These types of services are permitted while enrolled in palliative and may not be approved under hospice care.  Our palliative program does not replace your current provider; we act as a bridge of communication and resources while working alongside your current provider. 

Our qualified palliative representatives (physician, nurses, nurse practitioner, social worker) have many years of experience in helping to manage these uncomfortable symptoms.  We help make recommendations with you and your provider to create the best possible quality of life that are in alignment with your personal wishes. 

Our goal is to relieve suffering, provide symptom management, psychosocial support, advanced care planning, community resources, with regularly scheduled check-ins either by phone, in person or Telehealth.   We provide a compassionate and expert team that will listen and help recommend options based on your individual plan of care.   We are honored to be a part of your team….to walk alongside you on this journey…..to be your Anchor in the tides of life.
